STEP 2: Complete the Intake Form

Print and complete the "Counseling Intake Form" and bring it with you to your first appointment. A hard copy with original signature is required. If you do not bring the completed form we will have to reschedule your appointment. The "Permission for Exchange of Information" form is voluntary.
